The deficiency was noted through observation that the medical team failed to use prescribed pressure relieving boots on the resident as prescribed by her doctor. During observation, it was noted that the resident’s “pressure relieving boots were in the chair” when the nursing staff should have ensured she was wearing them. This problem occurred numerous times during one day of observation.
- Failure to Provide Residents Necessary Care and Services:
In a summary statement of deficiencies dated 08/07/2015, during an annual licensure & certification survey, surveyor’s noted the facility’s failure “to document, monitor resident behavior, and identify participating events to determine the underlying causes of combative behaviors for a resident with Dementia. The resident under review suffers with a severe impairment and “is totally dependent on staff for dressing, hygiene, bathing and incontinence care for urine/stool incontinence.
The concern is in response to incidences occurring from August 4-6, 2015 when a bruise on the resident’s right eye appeared even though “there was no documentation in the nursing notes pertaining to behaviors or participating in events in relation to the [resident’s] bruising.”
