In a summary statement of deficiencies dated 02/11/2016, a state surveyor made a notation during an annual licensure and certification survey concerning the facility's failure to "follow their wound prevention protocol and obtain a dietary consult for [one resident at the facility] reviewed for pressure sore interventions.” The state investigator also noted that the facility failed “to follow their wound prevention protocol and ensure that low air loss mattress was utilized as recommended by the manufacturer [for three residents at the facility] reviewed for pressure relieving interventions.”
In one incident, a resident at the facility who was “paralyzed below the waist and unable to turned/repositioned without assistance” appeared on 07/01/2015 with a “new skin breakdown [on the] left buttocks. The pressure ulcer Stage II [measured] 2.5 centimeters by 2.0 centimeters by 0.1 centimeters.” An additional Stage II pressure ulcer was found on the resident’s “right buttocks [measuring] 2.6 centimeters by 2.2 centimeters by 0.1 centimeters.” Eight days later on 07/09/2015 appeared with “an unstageable pressure ulcer” to their sacrum measuring “3.0 centimeters by 6.2 centimeters by 0.1 centimeters. A nutritional assessment was not conducted on [the resident until over a month later] on 08/12/2015.
In a separate summary statement of deficiencies dated 03/16/2016, a state investigator noted that the facility also failed to “consistently and accurately assess, and monitor a pressure ulcer after readmission to the facility for [a resident] reviewed for pressure ulcer evaluation and treatment.” This incident involved a 04/22/2015 readmission to the facility of a resident “after right knee revision surgery.” At that time, the resident “also had a right heel pressure ulcer.”
The state investigator interviewed the facility’s Nurse Practitioner/Wound Care Specialist on 03/16/2016 who stated that “he does not manage surgical incisions unless the surgeon requests him to [...and] denied being notified or managing [the resident’s] right heel pressure ulcer.”
The surveyor noted that the attending physician’s 05/18/2015 progress notes revealed that the resident’s posterior right heel wound now had “drainage serous fluid on a bandage. Slough covering 50% [of the resident’s] wound. No foul odor, no tenderness to palpation. No surrounding redness or swelling, though the borders of the wound were white, indicating maceration [softening of tissue after the skin’s death].”
