In one incident, the resident “smelled strongly of urine which [the facility’s infection control nurse] verified was coming from [the resident]. [The resident] stated her incontinence brief and not been changed all night.”
In a separate incident, another resident “was lying tilted sideways to her right side and resting on her elbow while trying to eat breakfast and drinking juice. [The resident] smelled strongly of urine. [The resident] stated the CNA help was poor, they do not come when you request assistance, the CNA’s do not show the residents they care about the resident well-being, and the resident’s wet the bed before a CNA arrives when requesting assistance for toileting.”
In a summary statement of deficiencies dated 01/24/2014, a complaint investigation is opened against the facility for its failure to “transfer one resident according to policies and procedures and failed to provide specific safety measures for confuse resident with numerous falls.” As a result of one of these failures a resident “was transferred improperly and sustained a fracture to the right humerus.”
