The investigation concerned the facility’s failure “to conduct thorough investigations for allegations of abuse. The facility failed to investigate bruises and injuries of unknown injuries.” This allegation concerns three separate incidences involving residents at the facility. In one incident, a resident stated that a Certified Nursing Assistant (CNA) “had thrown a cup at him.” In a prior incident occurring on 04/17/13 a resident reported to the “day shift staff, the night CNA was ‘very rough and rude’ to her.”
- Failure to Provide Residents an Environment Free of Accidents:
In a summary statement of deficiencies dated 06/17/2011, a complaint investigation was opened against the facility noting its failure “to provide supervision for a resident that has dementia and is at risk for falls.” This is in response to a failure to provide supervision for a resident that caused him to slip and fall on a wet dining room floor.
