This complaint is in response to a 93 year old female resident that suffered from “severely impaired” cognitive skills for daily decision-making. The resident “had falls since admission/entry or reentry or the prior assessment, and [the female resident] had 2 falls with no injuries since admission/entry or reentry or prior assessment. The registered nurse on duty “stated in part that she could not find documentation regarding updated interventions on [the resident’s] fall plan after [the resident’s] falls on 6/26/13, 7/11/13, and 9/10/13.”
