In a summary statement of deficiencies dated 04/04/2014, during an annual license and certification survey, it was noted that the facility failed “to recognize abuse for [2 residents] and failed to report abuse to the state agency immediately and within five days for [2 residents].” In addition, notations indicate that the facility “failed to investigate abuse allegation for [a resident].” “The facility failed to educate employees on the Elder Justice Act and failed to post a conspicuous notice regarding the Elder Justice Act. This failure has the potential to affect all 76 residents in the facility.”
