- Failure to administer oxygen therapy per physician order: “R3 was observed on 05/16/11 at 12:30 pm and 2:30pm, lying in bed. R3 was non -verbal. Surveyor observed R3 was receiving oxygen. R3 was receiving 1 liter per nasal canula. R3 is total dependent on staff for activity daily living. R3’s diagnosis are Legally Blind, [MEDICAL CONDITIONS], [MEDICAL CONDITION] Fibrillation, [MEDICAL CONDITION] Disorder, [MEDICAL CONDITION] and [MEDICAL CONDITION]. Surveyor prompted E3 (2nd floor unit manager) to check for oxygen on R3. E3 on 05/16/11 at 2:40 pm in the resident room stated,”R3 oxygen should be 2 – 3 liter. I will change the oxygen to correct liter.”
- Failure to discard expired medication: “During environmental tour on 05/19/11 at 11:55 am on the 3rd floor, surveyors observed in the refrigerator with E4 (nurse) Diazepam injectable with a expiration date of 06/10 and Bisacodyl
suppositories with an expiration date of 04/10. E4 stated, “the expiration date on the suppositories look like 04/2020.” The facility did not give a policy and procedure on expired medications.”
- Failure to discard expired enteral feedings: “During the environment tour on 05/19/11 at 11:45 am on the fourth floor, the surveyor observed inside the storage room on the west side one bottle of [MEDICATION NAME] with an expiration date of 04/01/11,one bottle of Two CalHN with a expiration date of 12/01/10 and one bottle of Nepro with a expiration date of 06/01/10. E2 (director of nursing ) on 05/19/11 at 12:00 pm on the fourth floor outside the storage room stated, “it should not be there”. E2 on 05/19/11 at 1:45 pm stated, “They are not even used them. I don’t know why they had them there.”
