After completion of the dressing change on the right hip, E4 and E5 turned R5 on her right side to treat the sacral wound. There was one dressing that covered the sacral and the right buttock wounds dated 12/18/10. 4 was asked if the dressing should have been changed on 12/21/10 instead of 12/22/10 since the dressing was dated 12/18/10 and the order was to be changed every 3 days. E4 stated that the dressing change was scheduled for 12/22/10.”
  • Failure to properly treat pressure sores: “R10 was observed on 1-20-12 at approximately 10:30am sitting in his wheelchair. R10 told facility staff that he will not allow surveyor to see pressure sore sites because it hurts and the doctor will be in shortly. Z2 (Wound Nurse, Nurse Practitioner) on 1-19-12 at 1:45PM explain that the reason for the order for R10 to see a Vascular Surgeon is because the wound was not healing and getting worse. Z2 went on to say that the characteristics of R10 wound was black with gray around the edges which could be a sign of gangrene. Z2 stated that further assessments are needed to get a definitive diagnosis or gangrene. E10 ( Staff Nurse) stated on 1-19-12 that she was waiting for the family first before making the appointment for R10 to see the Vascular Surgeon, so the order was not carried out.”
