In a summary statement of deficiencies dated 09/04/2012, during an annual licensure and certification survey, a notation was made of the facility’s failure “to document medical symptoms justifying the use of restraints for two residents and failed to release the restraint during activities.” This is a response to a review of a medical record that “does not document medical symptoms justifying the use of this restraint.” In addition, a nurse on charge noted that the resident needs to be reassessed “for this device as there is no medical diagnosis listed to justify its use.”
