This notation was a part of an interview and record review dated 12/27/09 where a resident was found “upset and crying.” When asked, “What was wrong [the resident] stated that someone was mean to her, when asked who, she refused to say.” However, the resident stated that “a staff member called her a ‘bit.. and a fat a..’ “
- Failure to Provide Proper Perineal Cleansing to Treat Incontinence:
In a summary statement of deficiencies dated 02/24/2014, a complaint investigation was opened against the facility indicating a failure “to provide proper perineal cleansing after an incontinence issue.” This is in response to a resident documents on 2-20-14 were a CNA (certified nurse aide) washed a resident’s “perineal area following urinary and fecal incontinence.” It is noted that one certified nurse aide “remove bowel movement (BM) from [the resident’s] perineal area using a back to front motion starting from [the resident’s] buttocks to [the resident’s] urethra (opening to the bladder).
In follow-up documentation, it was verified “that incontinence care should be performed by cleansing the perineal area from front to back” as a way to “prevent BM (bowel movement) from entering the Urethra (opening to the bladder).”
