This failure resulted in the facility not interviewing all of the witnesses and not sending a final investigative report to state agency within five days.” This is in response to a 5-29-14 incident when a daughter of a resident reported to the admissions manager that the resident’s roommate “made racial comments out loud while in their room. Both residents were separated; [and the resident allegedly making the racial slurs] was moved to another room.”
While the residents involved were interviewed and “determined that the abuse could not be substantiated”, the medical professional on duty “stated she did not interview [the resident’s] daughter or investigate the allegation to see if there were any witnesses present.”
- Failure to Provide Residents a Living Environment Free of Accidents and Hazards:
In a summary statement of deficiencies dated 6/13/02, it was noted that the facility “failed to maintain the building free of accident hazards.” This is in response to an annual licensure and certification survey or an observation was noted that “Hazardous compounds are not kept locked or attended in areas that house confused residents.”
