Both the federal government and the state of Illinois routinely update their nursing home databases to include recent inspections and surveys involving safety concerns, filed complaints, health violations and opened investigations. In a summary statement of deficiencies dated 08/27/2015, a notation was made by a state surveyor during an annual licensure and certification survey involving the facility's failure to "develop comprehensive care plans to address isolation precautions” for residents suffering with fistulas and bedsores. The deficient practice by the nursing staff at Sullivan Rehabilitation and Healthcare Center in part involve the resident who tested positive for VRE (Vancomycin-Resistant Enterococci) a highly contagious form of bacteria that harbors in human intestines that are highly resistant to most antibiotics.
The state investigator noted that during a 9:30 AM 08/24/2015 tour of the facility and observation of an isolation cart with supplies was seen outside the resident’s room in the hallway. The resident “was in isolation… for VRE of the urine.” The nurse providing care stated that the resident is an isolation but told two Certified Nursing Assistants, “if you are not touching her [the resident] you don’t need a gown, it is only contact isolation because of her urine.” However, the state investigator noted that the resident’s 07/14/2015 Care Plan “does not document a Care Plan or interventions for isolation or precautions of VRE.”
- Failures to Develop, Implement and Enforce Programs That Investigates, Controls and Keeps Infection from Spreading throughout the Facility
In a summary statement of deficiencies dated 08/27/2015, the state investigator noted the facility’s failure “to effectively decontaminate and disaffected mechanical lift after potential contamination [involving a resident] reviewed for infections.” The surveyor also noted that this failure “has the potential to affect for residents.”
The surveyor observed a Certified Nursing Assistant cleaning a mechanical lift after using the device on a resident infected with VRE by spraying “a small amount of Quaternary Ammonia Disinfectant” onto a cloth before beginning to clean the mechanical lift using only a small amount of the disinfectant before moving the machinery out into the hallway to be used on other residents.
The surveyor noted that the actions of the nursing staff failed to follow the facility’s 11/30/2009 policy titled: Multi-Drug-Resistant Organisms in Non-Hospital Healthcare Settings Items that reads in part: “Dedicate the use of non-critical items infected or colonized with VRE. (If such devices) are to be used on other residents, adequately clean and disinfect them first.”
