Disquieting Findings on the Patient Care at Advocate South Suburban Hospital

- Failure to properly remove expired medications: “The facility’s policy “MEDICATION ORDERING AND LABELING” dated 12/2008 states, “Multi-dose vials are to be dated on the day they first opened (initial use), discard 30 days from the date it was opened.” On 12/28/11 fifteen different medications were observed to have either been expired or not been dated when initially opened. On 12/28/11 at 2:40 pm, E8 (Licensed Practical Nurse) stated that the pharmacy delivery date is the date used when the medication is initially opened. They do not date and initial the medication on the day they first opened the medication.”
- Failure to follow isolation procedures: “E1 stated, “R4 is on contact isolation for [MEDICAL CONDITION] resistant staphylococcus aureus (MRSA) to the left lower leg.” During observation on 11/22/2010 at 11:00am accompanied by E1 (nurse), R4’s room was noted to have two garbage container with red isolation bags in them. There is no isolation sign/information posted on the entrance door of the room, and no gowns, gloves or masks available outside the room entrance either. So, surveyor interviewed E1 about the use of the red bags in the garbage containers and no available gloves, gowns or other items consistent with an isolation set-up.”
