[The resident] then hit visitor, [a nursing staff member] in the stomach area.” Another member of the nursing staff “grabbed [the resident with a] hand around [the resident’s] neck, pulled [the resident] back and then out of his wheelchair and onto the by ([the resident’s] neck holding [the resident] there for several seconds.” The resident indicated he could not breathe when he was grabbed around the neck again and “put into the ground.”

In a summary statement of deficiencies dated 02/21/2006, and incident investigation was opened concerning the facility’s failure to keep a resident “from setting a fire in the facility.” This is in response to a 2/6/07 incident where a resident was “upset about not wanting to be here and about the laundry and meals.”
