Left side hemiplegia from CVA. States she was grabbing for table. Lifted off floor by (Mechanical List) and put back to bed. Left leg having more pain and externally rotated.”

In a summary statement of deficiencies dated 08/22/2013, a complaint investigation was opened against the facility noting a failure “to document a medical symptom justifying the use of restraint and failed to have a restraint reduction program in place for [a resident].” This is in response to an observation of a resident placed with the seat belt around the waist without a “restraint reduction plan for [the resident’s] seatbelt use.”
