Seborg Terrace Ratings & Violations
Many residents in nursing homes and intermediate care facilities (ICF) lack the ability or capacity to maintain their own health and hygiene needs. These residents require an extra level of caregiving that can only be provided by competent medical staff. Without assistance from others, the residents can easily place their well-being in grave danger.
Our team of dedicated attorneys serve as advocates to many residents of nursing facilities throughout Illinois by providing legal services to our citizens who are most in need. We publish the most up-to-date information involving filed complaints, opened investigations, safety concerns and health hazards at ICFs, assisted-living homes and nursing centers statewide. Many families use this data to make informed decisions about these facilities before placing a loved one in a home that provides care.

Rockford Nursing Home Safety Concern
Both federal and state agencies regulate nursing homes and intermediate care facilities throughout Illinois to ensure all rules, regulations and laws are followed. The efforts of these agencies help maintain the health, safety and well-being of residents throughout Illinois. To ensure federal and state compliance, these agencies conduct routine inspections and surveys, and take action on every filed complaint and opened investigation involving allegations of harm, injury, neglect and abuse. The team of Rockford nursing home neglect attorneys at Rosenfeld Injury Lawyers LLC have found numerous deficiencies at Seborg Terrace. Some of these include:
- Failure to Provide a Resident Environment Free of Personal Harm:
In a summary statement of deficiencies dated 03/05/2014, during an annual certification survey, a notation was made of the facility’s failure “to ensure that a self-medication program stated the specific objectives necessary to meet the client’s needs as identified by the comprehensive assessment.” This is in response to a 22-year-old male’s individual service plan (ISP) dated 9/16/13 who functions in the mild range and is diagnosed with myopic dystrophy and acne.
In an incident occurring on 03/03/2014, the state surveyor observed the resident “locate his name in the medication carton and take his medication out.” According to the resident’s Medication Program dated 10/01/2013, the resident was given the authority to self-medicate. In an interview with the facility administrator occurring on 03/04/2014, the administrator “stated if she had done the assessment it would have been different, that the data reflects where [the resident] is at in the program is where it needs to be but the [resident’s] assessment is not.
- Failure to Adequately Train Medical Staff Members:
In a summary statement of deficiencies dated 04/03/2013 during an annual licensure and certification survey, notations were made of the facility’s failure to ensure that residents “receive medications that staff demonstrate skills and techniques necessary for each client to whom they are responsible.” This is in response to an observation where a member of the team dropped medicine to the floor and improperly disposed of the drug in a trash container.
Through inadequate training, the nursing staff did not follow written policies that indicates “that wasted medication will be disposed of in accordance with the rules and regulations of the Federal Drug Enforcement Administration to render them non-retrievable contained in a non-palatable substance.”
