    In a summary statement of deficiencies dated 08/21/2012, during an annual licensure and certification survey, it was noted that the facility “failed to document the medical symptom justifying the use of restraints, obtain consents and develop plans of care for the use of adult reclining chairs for 13 residents.” This is in response to findings that 13 residents were observed in adult reclining chairs that were not assessed to be restrained in a way as to “keep them in the dayroom for close observation and prevent them from falling while alone in the rooms.”

  • Failure to Provide Residents a Clean and Sanitary Environment

    In a summary statement of deficiencies dated 03/07/2015 a complaint investigation was opened based on the facility’s failure “to schedule the same number of laundry staff to work on the weekends and failed to maintain the washing machine in good working order.” As a result, the surveyor observed a “pile of dirty residents clothing a foot away from the ceiling, bags of dirty linens up to the ceiling and five carts overflowing with dirty sheets and pads. There were no clean linens noted on the linen carts in the laundry.”
