Troubling Discoveries Regarding Patient Care at Rosewood Care Center Northbrook

A nursing home survey carried out by private health inspection firms and the State Department of Health, discovered poor patient care, inadequate patient supervision and cases of abuse which can be construed as patient negligence. • Failure to train staff in emergency procedures: “Based on interview and record review, the facility failed to ensure that all staff have knowledge of facility emergency procedures. On 6/12/12 at 11:00 AM, E16, Certified Nursing Assistant (CNA) was interviewed by the survey team regarding her responsibilities during a weather emergency, such as a tornado. E16 paused for a minute and then stated, “To be honest I don’t know”. At 11:15 AM, an E5, Registered Nurse (RN) was interviewed regarding fire emergencies. E5 was asked to tell the survey team the locations of the emergency fire pull boxes on his floor and to locate the facility’s disaster manual. E5 failed to answer either of the questions correctly and had to look around the floor to find the alarm pull boxes and asked another staff person regarding the location of the disaster manual.”

• Failure to provide adequate supervision for residents: “R3’s Incident Report indicated that on 11/4/11 at 3:15 PM, R3 was found in the bathroom lying on the floor, with her buttocks on the wheelchair leg rest, and her left hand holding on to the bathroom rails. R3 was found in the bathroom alone without staff supervision, after her wheelchair alarm triggered. On 1/20/12 at 2:55 PM, E5 (nurse) said that on 11/4/11, she found R3 sitting on her wheelchair foot rest in her bathroom, after her chair alarm sounded. E5 said R3 was by herself. R3’s 11/23/11 incident report indicated that at around 5:10 PM, R3 was again found in the facility basement, lying on the floor. R3 fell from her wheelchair because she was on the toilet without any staff supervision, and cannot be left by herself in areas she shouldn’t be at, unsupervised. Facility’s incident investigation of R3’s 11/23/11 fall incident showed that no staff was with R3, when R3 went to the basement by herself. R3 sustained laceration at the left forehead and hematoma, which resulted to her hospitalization on [DATE]. R3’s hospital record printed on 11/26/11 indicated that she had contusion, abrasions, and subgaleal hematoma.”
