This investigation is in response to a June 13, 2015 incident where an “isolation cart outside [the resident’s] room had no isolation gown on it. [Two] CNAs (Certified Nursing Assistants) transferred [the resident] to a wheelchair without wearing PPE (Personal Protective Equipment) gowns.”
- Failure to Maintain a Residential Environment Free of Abuse:
In a summary statement of deficiencies dated 12/12/2013, a complaint investigation was opened concerning the facility’s failure “to ensure that an allegation of abuse was thoroughly investigated.” This is in response to an incident occurring on 12/11/13 where a certified nursing assistant stated, “I reported an allegation of abuse for [a resident] earlier this week. [The resident] said that the CNAs were not washing her and that they tell her to shut up. I told my nurse [a staff LPN] and I told [a staff social worker].”
However, the social worker indicated that “no one reported any abuse allegation to me.” The resident later complained that she “was red under her breasts and in her groin because she had not been washed.”
In a separate summary statement of deficiencies dated 10/03/2013, “an immediate jeopardy situation was identified” when a resident “began displaying verbal and physical aggression toward other residents.” Even though the resident was removed from the nursing home, investigators noted that “the facility remains out of compliance at a severity level 2 due to the need to assess all residents for their potential for abuse, review and revise care plans with behavioral interventions, and evaluate the implementation of their policy and procedure for aggressive behavior.”
