In a summary statement of deficiencies dated 03/11/2015, during an annual licensure and certification survey, notes were made of the facility’s failure “to follow wound care product recommendations for product usage and removal. The facility failed to cleanse the wound in a manner to avoid potential trauma to the surrounding tissue and discomfort to the resident.” This notation is made in response to a March 2015 incident involving a resident diagnosed with diabetes mellitus, stress incontinence and multiple sclerosis.
- In a separate summary statement of deficiencies dated 01/03/2008, a complaint investigation was opened concerning the facility’s failure to “identify open areas on [a resident’s] sacral area.” This is in response to a resident who was diagnosed with congestive heart failure, dementia and diabetes. The notation indicates that on 12/14/07 the resident “was readmitted to the facility from the hospital with no open or reddened areas present. However, by 1/2/08, a CNA at the facility told a staff nurse that the resident’s sacral area was “getting very red.” When nurses were interviewed concerning the problem, one nurse indicated that “she saw four open areas on [the resident] and reported to the nurse on 12/25/07.” However, the “nurse did not document it.”
