In a summary statement of deficiencies dated 10/15/2015, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "follow their policy for isolation for [a resident at the facility] reviewed for isolation and infection control practices during the provision of care.” The deficient practice by the nursing staff at Renaissance Care Center was first documented at 9:45 AM on 10/14/2015 when a resident “was being repositioned in bed by [two Certified Nursing Aides (CNAs)]. Both [CNAs] were wearing gloves during the procedure, but no gowns.”
During an interview with one of the Certified Nursing Assistant at 1:20 PM on the same day, it was revealed that “we always wear gloves to perform direct care for a resident in isolation and we change [the resident’s] bed linen. I don’t know why we didn’t today. If [the resident] has a bladder spasm and his sheets are wet, we stop, put on a gown and gloves to continue [the resident’s] care.”
The state investigator interviewed the facility’s Director of Nurses at 1:35 PM on the same day who stated that “it would not be an acceptable standard of practice for staff not to wear gowns and gloves during direct resident care for a resident in contact isolation.” The investigator noted that the actions of the Certified Nursing Assistant’s failed to follow the facility’s 01/07/2015 policy title: Infection Control: Isolation that reads in part:
“contact Precautions for Residents that are infected with microorganisms that can be transmitted by direct contact with the resident or indirect contact with environmental surfaces or resident care items in the resident’s environment. Wear gloves and wear a gown for all interactions that involve contact with the resident or potentially contaminated items in the resident’s environment.”
- Failure to Maintain Accurate Drug Records and Properly Label/Mark Drugs and Other Similar Products According to Accepted Professional Standards
In a summary statement of deficiencies dated 06/09/2015, a complaint investigation against the facility was opened for its failure to "ensure staff monitor all residents and confirm they had taken medication being administered to them.”
The deficient practice was first noted on 05/31/2015 when the resident’s Nurse’s Notes documented that the resident “was sent to the hospital” where a staff noted the resident’s “purse contained a large amount of various pills. A Hospital Discharge Summary dated 06/03/2015 documents that during the resident’s hospitalization, it was noted that her purse was packed with pills, presumably from hoarding pills during medication passes.”
