The notes document that a resident ‘has been having stools, and has been placed in isolation. An order to “obtain stool for C. diff’ was written 1/28/13.”
“All four residents were observed to be in isolation.” Nurses Notes recorded on 1/24/13 indicate that the resident’s “room was observed. There were numerous dark areas noted on the carpeting around mats and in front of the bathroom doorway. Dark spots were also observed around the bed in [the resident’s] room.” On 1/28/13 “housekeeping was observed vacuuming in [the resident’s] isolation room, without wearing personal protective clothing, and carrying the vacuum out when finished and placing it on the housekeeping cart without bagging or cleaning.”
- Failure to Provide Residents an Environment Free of Accident Hazards
In a summary statement of deficiencies dated 01/04/2011, a complaint investigation was opened noting that their facility “failed to assure that residents are transferred properly to prevent accidents.” This is in response to a review of the facility’s incident log where a resident “was identified as having two incidences where he receives skin tears while receiving care. On 11/15/10 he received a skin tear on the back of his upper left arm, while being transferred by staff. On 12/23/10 he received a skin tear on the back of his right arm while staff prepared him for a shower.”
