Troubling Discoveries Regarding Patient Care at Presence Villa Scalabrini Nursing & Rehab

According to data from nursing home surveys conducted by the Illinois Department of Health other agencies, there have been numerous episodes of poor patient care, and medication errors at Presence Villa Scalabrini Nursing & Rehab which can be construed as nursing home negligence. Our nursing home attorneys reviewed data compiled by ProPublica and found the following conditions that can be construed as nursing home negligence:

  • Failure to develop a comprehensive care plan for residents: “During the initial tour of the facility on 2/7/12 at approximately 10:20am, R1 was observed in her room, in bed. There was an isolation precaution sign posted on R1’s door. A nurse was present and stated that R1 is on contact isolation for ESBL of the urine. Nurses Notes dated 2/4/12 states R1 is maintained on contact isolation for ESBL of the urine. R1 has a [DEVICE] (wound vac) of sacral and ischium wounds. The Care Plan does not address the urine infection, the contact isolation status nor the use of wound vac. Nurse’s Notes dated 2/5/12 states R1 has a double lumen peripherally inserted central catheter (PICC line). The care plan does not address the presence of a PICC line. The non-comprehensive care plan was discussed with E1 (Administrator) and E2 (Director of Nursing) on 2/8/12 at approximately 4:15pm. Neither E1 nor E2 provided any explanation.”
  • Failure to properly assess and treat skin breakdown: “On 2/7/12, at 3pm, R13 was lying in bed; heels were not elevated off the bed. R13 was not wearing heel protectors of any kind. The December, 2011 Physician Order Sheet documents an order to suspend both heels on pillows. Nursing Admission Assessment and Foot assessment dated [DATE] both note redness to R13’s heels. Braden Scale dated 12/3/11 has a score of 11, which is high risk for acquiring pressure ulcers. No preventative interventions were recommended. Care plan dated 12/2/11 addresses R13’s sacral pressure ulcer, but does not address the redness to both heels. During a phone interview with Z2 (Attending Physician) on 2/10/12 at approximately 12:25pm, Z2 stated that he was not aware that R13 had bilateral heel redness on 12/2/11. Z2 stated had he known, he would have ordered preventive measures, such as lamb ‘ s wool, to cushion R13 ‘ s heels, close observation of the redness, or medication to improve circulation to R13 ‘ s legs. There was a 2 week delay in medical intervention which resulted in skin breakdown progressing from redness to deep tissue injury.”
  • Failure to prevent serious medication errors: “Review of R27’s Diabetes Records flow sheet notes on 1-19-12, there was no results of her blood glucose. On 1-26-12 at 12:00PM (noon) R27’s results were 282 and 2 units of regular insulin as given. According to the insulin scale R27 should have received 5 units of regular insulin.Review of R26’s diabetic record flow sheet on 1-30-12 at 6:00AM her blood glucose level was 273. The diabetic flow sheet notes no insulin was administered, (an error of omission). According to her blood glucose sliding scale R26 required 4 units of regular insulin. Review of her records none was given.Review of R42 diabetic record flow sheet on 1-10-12 at 4:00PM her blood glucose was 181. One unit of regular insulin was given. According to her regular insulin scale R42 should have been administered 2 units of regular insulin (an error of the incorrect dose).”
