During observation, it was noted that the “use of repositioning device was not being used correctly. They should have used 2 pieces of nylon fabric one on top of the other. [This is because] 2 pieces of nylon fabric slide against each other reducing friction and shearing.” Nurses notes dated 07/18/14 indicated that the resident “had shearing to the right buttock. The wound care physician would be contacted.”
- Failure to Develop Policies on Use of Physical Restraints:
In a summary statement of deficiencies dated 01/16/2014, during an annual licensure and certification survey, a notation was made of the facility’s failure “to assess the need to use a safety belt as a physical restraint.” This notation is in response to a 1/14/14 incident indicating that a resident was wearing a restraint belt around the waist as an intervention to avoid falling.
