In a summary statement of deficiencies dated 06/04/2015, a state surveyor made a notation during an annual licensure and certification survey concerning the facility's failure to "ensure a resident received all medications as ordered by a physician.” The failure by the nursing staff at Premier Rehabilitation and Skilled Nursing Center involved a resident who did not receive their prescription Metformin on 03/16/2015 and did not receive other medications on 04/02/2015 and 04/03/2015.
The surveyor interviewed a Licensed Practical Nurse in regards to the facility’s protocol “for what nurses should do if a resident’s medication was not available during a medication pass.” The Licensed Practical Nurse stated that “the nurse should contact the charge nurse who would get the medication from the contingency box, if the medication was not in a contingency box, then the pharmacy should be called to deliver the medication to the facility.”
When the investigator interviewed the facility’s Director of Nursing on 06/03/2015 about the medication withheld from the resident, the Director stated: “that he was not aware that the resident’s medication was held because it was not available [on those three dates].”
- Failure to Provide Care by a Qualified Individual According to Each Resident’s Written Plan of Care
In a summary statement of deficiencies dated 06/04/2015, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility's failure to "ensure that services provided are arranged by the facility and are provided by qualified persons in accordance with each resident’s written Plan of Care.” The failure by the nursing staff at Premier Rehabilitation and Skilled Nursing Center involved three residents at the facility.
In one incident, a resident with “a Behavioral Management Plan instructing facility staff to use two Certified Nursing Assistants approach during all cares.” Observations by the state investigator were made and noted that “this Care Plan intervention was not followed.” Additionally, “observations were made when [the resident’s] right ankle and foot were not floated off of the mattress as instructed in her Plan of Care” to ensure she did not develop a bedsore.
