During a review, it was observed in the Nurse’s Notes, Treatment Administration Record (TAR) and physician record dated 2/22/13 “that the resident has a pressure ulcer on the right heal but no pressure ulcer on the left heel.” However, on 4/16/13 the treatment nurse on duty indicates that the resident “was noted with a skin breakdown on the left and right heal” showing that the resident has developed a facility-acquired pressure sore on the left heel.
