In a summary statement of deficiencies dated 01/14/2016, the state agency surveyor noted the facility’s failure to “follow its Abuse Policy by not reporting an allegation of abuse immediately to the Administrator.”
The deficient practice was first noted in the state investigator’s findings after reviewing reports of a resident at 1:30 AM on 03/02/2015 who “became combative during cares as documented on the Resident Abuse Investigation Report Form… [that documents a] Certified Nursing Aide (CNA) alleged that [another CNA] raised her fist to the resident and told the resident she was not to hit her again.” The state investigator noted that “this allegation was not reported until 6:30 AM”, nearly five hours after the incident occurred.
In an interview with the facility’s Administrator at 4:00 PM on 01/13/2016, the Administrator stated: “they review reporting of abuse on a routine basis [...and] the steps to report abuses on the back of [employee] name badges.”
The investigator reviewed the Abuse Prevention directions printed on the back of every Care Staff’s name badge that reads:
“Report any unusual behavior or occurrences immediately to your supervisor.”
In addition, the Abuse Prevention Program Facility’s Procedures note under section 5 that:
“Employees are required to report any incident, allegation or suspicion of potential abuse, neglect… they observe, hear about, or suspect to the Administrator or an immediate supervisor who must then immediately report it to the Administrator.”

In a summary statement of deficiencies dated 01/14/2016, the state investigator noted the facility’s failure “to ensure that staff respond in a timely manner to the resident’s request for assistance.”
The deficient practice was first noted in the state investigator’s findings “during the residents’ Quality of Life Group Interview held on 01/13/2016.” The residents at the Group Interview “were questioned about using call lights for staff assistance and the length of the weight for the staff response.”
“One resident responded that they do use the call lights to notify staff of their needs and need for assistance for transfer, toileting, dressing and other assistance.” During the interview, five residents stated: “they have waited an hour for assistance at times [...and two resident stated] they know it was an hour because they were watching the clock.” Two resident stated, “they have cried due to having to wait in soiling themselves.”
