Downers Grove Nursing Home Resident Safety Concerns
On an annual basis, the federal government and the state of Illinois routinely update their nursing home databases in an effort to reflect the most currently opened investigations, filed complaints, safety concerns and health violations on facilities throughout the United States. The information is then posted on numerous websites including Medicare.gov, through their comparative analysis star rating summary system.

- Failure to Allow Residents the Right to Participate in Planning or Revising Their Care Plan
In a summary statement of deficiencies dated 07/29/2015, a complaint investigation against the facility was opened for its failure to "revise, update and implement a fall Care Plan with intervention to provide a safe environment.” The deficient practice by the nursing staff at Oak Trace Senior Living Facility involved one resident “who has a history of falls and injuries.”
The resident was readmitted to the facility after surgery on the right hip after a fall occurred on 07/08/2015. By 07/15/2015, the facility assessed the resident “to be at risk for fall.” A review of their 07/24/2015 Care Plan revealed “there were no interventions added to [the resident’s] fall Care Plan after returning to the facility” following their stay in the hospital.
During an interview with the resident occurring on 07/23/2015 at the nurses’ station, the observer noted that the resident “was alert but would not speak when spoken to and did not elicit conversation.” The investigator noted that the resident “was unable to recall the fall on 07/08/2015” or a previous fall that occurred on 07/21/2015. The surveyor also noted that a review of the resident’s Care Plan showed “there were no interventions [to minimize the potential of falling] added until 07/22/2015 after [the resident] sustained a fall from the bed on 07/21/2015.” It was also noted that even though the staff added a bed alarm to the resident’s bed to alert the staff that the resident was attempting to get out of bed, “the bed alarm sound was very low and could not be heard from the hall.”
In a separate summary statement of deficiencies dated 09/21/2015, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "develop interventions and revise care plan interventions for a resident’s change in skin.” This deficient practice by the nursing staff at Oak Trace Senior Living Center involved a cognitively impaired resident who requires the extensive assist of two staff persons for bed mobility and transfers.
The investigator noted that the resident’s Stage II open area to the left buttocks cheek was documented on 09/22/2015 after the September for 2015 Wound Notes revealed the resident had “developed an open pressure area to the left inner buttocks area [measuring] 0.8 centimeters by 1.0 centimeters by 0.1 centimeters that were not present on admission.” While the resident’s “Plan of Care for alteration in skin integrity [was] initiated on 08/21/2015, [it] showed no new interventions after 08/21/2015” to provide measures to allow the open wound to heal.
