Troubling Discoveries Regarding Patient Care at Norridge Healthcare & Rehab Centre

A survey conducted by the State Department of Health and diverse private health inspection firms discovered some series of poor patient supervision, repeated falls and inadequate pain relieving policies for admitted patients. The survey conducted by ProPublica—a private inspection firm—was reviewed by our nursing home attorneys in Rosenfeld Injury Lawyers and episodes of poor supervision which could be construed as nursing home negligence were found:

  • Failure to implement interventions to prevent falls: “Review of R1’s unusual occurrence report dated 1/16/10 at 3:45 AM, shows, that R1 was found on the floor next to bed laying on the right side. Per report, R1 stated, “I slid down from bed.” R1 complained of pain on the right shoulder, was able to move the right hand with limited range of motion, able to move left hand, able to move both lower extremities. R1 sustained bruising on the back of the right hand and back of the left lower arm. The fall occurrence report investigation report under analysis of fall shows that the “Bed alarm found not functioning.” The same report also shows that R1 had history of falls. During interview held on 3/1/10 at 1:35 PM, E3 (CNA) stated that she was the staff who found R1 sitting on the floor beside the bed on 1/25/10 at around 2:20 AM. E3 stated that she did not see any bed alarm hanging at the bed to indicate that R1 had a bed alarm in place prior to the incident of fall. E3 also stated that when she found R1 on the floor on 1/25/10, there was no sound coming from an alarm.”
  • Failure to ensure call lights are within reach of residents: “R32 was sitting in a wheelchair at the right side, by the foot of the bed. The call light was on the left side of the bed under the pillow. R32 was unable to reach the call light. At that time, E10 (Assistant Director of Nursing) stated that R32 can’t get to it. R33 was sitting in a wheelchair near the foot of the bed. The call light was draped on the wall. R33 was unable to reach the call light. R34 was in bed with the call light attached to the side rail. The call button was near the floor. The call light was inaccessible for R34.”
  • Failure to provide pain management during wound treatment: “E3 prepared to initiate wound treatment to R9 and was heard asking E4 (Nurse) whether R9 received pain medication. Surveyor did not hear E4’s response. E5 (Certified Nurses Aid) was present, suspending and supporting R9’s left leg during treatment. Upon beginning the treatment, E3 said to R9 “let me know if you feel any pain.” While cleansing the left heel wound R9 was observed to exhibit facial grimace and made a jerking movement of the left leg. E5 told surveyor that the jerking movement of the leg is because R9 is in pain. E3 continued to treat the wound with no regard for the facial grimace and Shying behavior. On 1/11/12 at 12:10pm E4 told surveyor she administered the medication to R9 to minimize pain during wound treatment. E4 went on to state that she always medicate R9 one hour prior to wound treatment. R9’s MAR shows no [MEDICATION NAME] administered for pain management during period of 1/6/12 to 1/10/12.”
  • Failure to obtain timely medical care: “R1 had an unwitnessed fall on 1/25/10 at 2:20 AM. Five hours after the fall incident, the facility consulted another physician who was in the facility, after three prior attempts to contact the attending physician without response. R1 had a STAT x-ray of the femur after this consultation. The x-ray result revealed an oblique supracondylar [MEDICAL CONDITION] femur. R1 was sent to the hospital and was admitted with [DIAGNOSES REDACTED]. This failure to actually contact a physician in a timely manner and to assess on-going changes resulted in the delayed evaluation and treatment of [DIAGNOSES REDACTED].”
