Many individuals who suffer developmental disabilities experience significantly compromised intellectual functioning and impairments to adaptive behaviors. In many cases, their limitations are attributable to epilepsy, cerebral palsy or other conditions including mental illness that necessitates ongoing assistance to maintain their health, hygiene and well-being. These individuals often require special services that can usually be provided at intermediate care facilities (ICFs) under the care of specially trained medical staff. However, many facilities lack the necessary training or are overcrowded or understaffed, which results in the resident receiving substandard care.
Our dedicated lawyers provide legal remedies for every developmentally disabled individual in Illinois. In an effort to help families make an informed decision, we publish current information detailing safety concerns, health hazards, opened investigations and filed complaints against intermediate care facilities statewide, including those that are documented at Milestone Elmwood Heights. Many individuals and family members who must place a loved one in a home that provides quality care use this information to make the best informed decision to ensure their safety and well-being.

Rockford Nursing Home Safety Concerns
As an intermediate care facility, Milestone Elmwood Heights is required to follow laws, rules and protocols regulated by state and federal agencies. Throughout the year, state surveyors and inspectors perform routine assessments and evaluations of the facility to ensure they operate at full compliance under the law. The skilled nursing home neglect attorneys at Rosenfeld Injury Lawyers have uncovered serious safety concerns, health hazards and deficiencies at the facility that include:
- Failure to Provide a Safe Resident Environment:
In a summary statement of deficiencies dated 07/01/2015, an incident investigation was opened against Milestone Elmwood Heights ICF involving the facility’s failure to ensure that the individual program plan (IPP) for a client was used to ensure proper usage of a gait belt. As a result of the facility’s failure, the resident “fell and sustained a laceration and intracranial bleed.” Emergency medical services were called after a resident was “found sitting on the floor outside another peer’s suite (bedroom). [The resident] was observed with blood on the right side of his forehead with a laceration received from an apparent tripping over a protective floor mat.
After medical evaluation the resident was “admitted to the hospital’s ICU (intensive care unit) with a primary diagnosis of “Anticoagulant Induce Blood Clotting Disorder-bleed in the brain.” An investigation conducted at the facility “identified that [the resident] may have tripped over a floor mat that was in his peer’s bedroom.”
- Failure to Implement Individual Program Plans for Every Client:
In a summary statement of deficiencies dated 05/21/2015, during an inspection of care and certification and licensure survey, a notation is made of the facility’s failure to ensure proper training of the staff to use correct techniques and properly administer medications at the facility. This is in response to a surveyor’s finding that “monthly weights [of residents] have not been documented as ordered. [The assistant director of nursing] said she has informed administration of the missing weights, and that the weighing should be scheduled around the same day each month, for consistent monitoring.” Weighing every resident is important, especially for those who require weight management.
