After medical evaluation the resident was “admitted to the hospital’s ICU (intensive care unit) with a primary diagnosis of “Anticoagulant Induce Blood Clotting Disorder-bleed in the brain.” An investigation conducted at the facility “identified that [the resident] may have tripped over a floor mat that was in his peer’s bedroom.”

In a summary statement of deficiencies dated 05/21/2015, during an inspection of care and certification and licensure survey, a notation is made of the facility’s failure to ensure proper training of the staff to use correct techniques and properly administer medications at the facility. This is in response to a surveyor’s finding that “monthly weights [of residents] have not been documented as ordered.
