Concerning Findings Regarding Patient Care at Manorcare of Oak Lawn East
According to the data gathered from the nursing home surveys conducted by the Illinois Department of Health and other survey agencies, there have been noticeable signs of abuse and neglect at the Manorcare of Oak Lawn East, which can be construed as nursing home negligence. On examining the records, our nursing home attorneys discovered the following:
- Failure to provide consistent catheter care: “R3’s re-admission assessment of 10/24/11 reflects that R3 was readmitted from the hospital with an indwelling catheter. Review of multiple care plans initiated 10/25/11, 10/26/11 and 11/5/11 reflects no care plan for R3 having an indwelling catheter. Additionally, there is a care plan for Urinary/Bowel incontinence initiated 11/5/11 which does not mention an indwelling catheter. The first mention of R3’s catheter is in a nursing note dated 12/3/11, at 10:50 am, which indicates the catheter was draining clear yellow urine. On 12/4/11, at 10:00 pm, another note again indicates the catheter was draining well. A nursing note from 12/5/11 at 10:00 pm reflects that the catheter was draining amber-colored urine. This note indicates catheter care was rendered, but does not state specifically what was done.”
- Failure to develop fall prevention interventions to address repeated falls: “Review of facility incident reports indicated that R 1 was involved in 4 fall related incidents: 1. 3-14-11 at 12:45 am, R 1 was found on the floor on the left side of the body, at the foot of the bed. R 1 was taken to the local hospital. R 1 sustained a fractured Right Hip. R 1 returned to the facility on [DATE]. 2. 3-22-11 at 8:30pm, R 1 was found kneeling on the floor next to the bed. 3. 3-26-11 at 8:29 am, R 1 was found in the hallway, attempted to get out of the wheelchair and fell on to the floor. 4. 3-26-11 at 10:40 am, a CNA(Certified Nursing Assistant) was trying to transfer R 1 back into the chair after standing, R 1 backed up slipped, hit his head on the footboard of the bed. This incident resulted in R 1 going out again to the hospital. R 1 did not return to the facility. Further review of the fall careplan does not show any additions or revised interventions associated with each fall occurrence.”
- Failure to accurately assess risk of developing pressure sores: “Based on record review and interview, the facility failed to take into account major risk factors present for R3 in determining the overall risk of developing pressure ulcers. The facility also failed to follow its’ protocol for assessment and monitoring the skin of R3 who was deemed at low risk for pressure ulcer development. These failures resulted in R3 being assessed as low risk for development of pressure ulcers rather than at high or moderate risk. R3 was treated with a lower level of interventions and monitoring of skin by less skilled staff than if at high risk. These failures to accurately assess and monitor R3’s skin integrity resulted in R3 developing deep tissue injuries to bilateral heels 2 weeks after admission to the facility.”
