Disturbing Discoveries Regarding Patient Care at Manorcare of Northbrook

According to data from nursing home surveys conducted by the Illinois Department of Health and other private agencies, there have been numerous episodes of poor patient care, repeated falls and inadequate communication equipment at Manorcare of Northbrook which can be construed as nursing home negligence. Our nursing home attorneys reviewed data compiled by ProPublica and found the following conditions that can be construed as nursing home negligence:

  • Failure to utilize interventions to prevent falls: “R1, upon admission, was identified as a high fall risk. Initial interventions included monitoring, call light device, and physical therapy. Review of nurses notes dated 12-29-09 thru 2-18-10 does not show documented evidence that the alarms were being implemented. On 2-18-10 at 6:00pm, R1 was found lying on the floor in the middle of the room. Incident Report dated 2-18-10 indicates that R1 told staff that he “stood up to go to the bathroom, lost balance and fell.” The fall resulted in R1 complaining of right hip pain. R1 was taken to the local hospital for evaluation and treatment. R1 was admitted with a right hip fracture. R1 expired on [DATE].”
  • Failure to provide adequate supervision for residents: “Review of R2’s closed medical record indicates R2 has been in and out of the facility since 2/1/09. Throughout R2’s medical record from the hospital, it reads that R2 has had numerous falls while at home. On 8/3/09, per facility incident report, R2 fell, sustained a slit to his forehead, and was sent to the hospital. Per Z2’s (nurse practitioner) note, dated 8/6/09, R2 sustained a cut in his forehead after falling and hitting his head. R2 was sent to ER (emergency room) for eval., sutures needed. Review of facility incident report and E4’s (licensed practical nurse) note: R2 fell again on 9/13/09 while at the facility for a respite stay. This fall required R2 be sent out 911 to the emergency room. Review of R2’s hospital record dated 9/13/09 reads per CT scan (Computerized Tomography)… There is an impacted fracture involving the right occipital condyle with displaced fracture involving the lateral aspect of the right C-1 (cervical) vertebra. On 9/14/09 another CT scan was performed which reads: Head CT, Impression: Left frontal bone fracture which extends through the anterior and posterior walls of the frontal sinus a swell as the left orbital roof. Cervical Spine CT: Impression: Comminuted fracture involving the right occipital condyle. Right Transverse process fracture of C1.”
  • Failure to provide working call lights in resident bathrooms: “During environmental tour with E1 (Administrator), E3 (Maintenance Supervisor), E4 (Housekeeping Supervisor) on 12/08/11 at 1:30 p.m., call light above resident community wash room did not work when call light string was pulled.”
