- Failure to render incontinent care as scheduled “On 3/29/12, R1 was observed sitting in his wheelchair, and with a fecal odor from 10:15 AM to 1:45 PM. No toileting or incontinent care was made during that length of time, which E5 confirmed. At 1:45 PM, E5 (Certified Nursing Assistant/ CNA) transferred R1 to bed for incontinent care. R1 had a huge bowel movement that already overflowed from his incontinent brief. R1’s buttocks, groins and scrotum were all red and raw looking. R1 stated that there’s a burning sensation on those parts of his body.”
- Failure to implement effective fall protection interventions: “On 3/29/12, review of R4’s chart indicated that R4 fell on [DATE], 12/16/12, 12/21/12 and 12/24/12. All incidents happened inside R4’s room unwitnessed. Progress note [REDACTED]. Despite those interventions, R4 continued to fall. On 3/30 /12 at 10:40 AM, E2 (Nurse Supervisor) stated, R4 came from another rehab facility but R4 did not improved over there. When R4 came to this facility R4 started to get better functionally. R4 received physical and occupational therapy. Because R4 improved with her mobility, R4 was getting up by herself, causing her fall incidents. E2 added, facility provided personal alarm, bed sensor, low bed position and moved R4 closer to the nurse station. “
- Failure to keep the rate of medication errors below 5%: “On 10/18/11 at 11:29am, E13 (Licensed Practical Nurse) administered [MEDICATION NAME] swish and swallow suspension to R1. E13 held the medicine cup at eye level and poured the medication into the cup. E13 stated the amount poured was 5 ml. As E13 was about to administer the medication to R1, E13 was asked to verify the amount poured. Using a 6 ml syringe, E13 drew up the medication which was measured at 4.2 ml. E12 discarded the 4.2 ml and drew up 5 ml of [MEDICATION NAME] into the syringe and administered the medication to R1.”
- Failure to follow a program that controls and keeps infection from spreading: “On October 17, 2011 during the initial tour at 9:45 am, E5(Certified Nursing Assistant) was in R13’s room changing bed linens. R13 is on contact precaution for, [MEDICAL CONDITION]. Difficle ([MEDICAL CONDITION]). E5 wore a pair of gloves but was not wearing a gown. Some parts of E5’s clothing were exposed and were touching R13’s bed, pillows and bedside table and soiled linen.
