- Failure to keep the rate of medication error below 5%: “On October 6, 2011 at 9:11am, E3, Registered Nurse administered Potassium 20meq(millequivalent) 2 tablets, which equals 40meq to R23 by mouth. The POS date 10/2/2011 list for R23 to receive [MEDICATION NAME] 20mg by mouth twice a day, not 40mg at one time.”
- Failure to implement a program that prevents spread of infection: “On 10/4/11 at 10:00 am, R9 had a Port-A-Cath (Central Intravenous Catheter used to administer treatment/medications through the vein) which contained a Huber needle (needle to inject medication/treatment/fluids into the vein) located in the right side of her chest. The protective dressing for the Port-A-Cath for R2 was dated 9/25/11 (10 days old). The physician’s order [REDACTED].’s policy is to change the central line dressings once a week on Sunday.”
- Failure to maintain the dignity of residents: “On 10/4/11 and 10/5/11, the following staff walked in residents rooms without knocking and requesting permission to enter while the residents were inside. On 10/6/11 at 9:50AM, R9 was sitting in his wheelchair in the therapy room with his urine bag uncovered hooked at the back of his wheelchair. He stated that he was waiting for staff to bring him back to his room.”
