All stated that the front door is guarded, and the only residence with Clarence can leave the building.” The nurse in charge stated that the resident “must have left the facility when the receptionist was talking to someone and did not see [the resident] go out with other residents or visitors. The front door is not alarmed when the receptionist is on duty. All other door alarms were checked and were working.”
