Disturbing Discoveries Regarding Patient Care at International Nursing & Rehabilitation Center
A nursing home review conducted by the Illinois Department of Health and other private health inspection firms discovered different episodes of poor patient care and abuse at the health center which could be interpreted as nursing home negligence. - Failure to provide sufficient notice before discharging or transferring a resident: “R7’s nurse’s notes dated 10/28/2011 5 P.M. documented, R7 in dining room agitated struck staff member. Moved R7 from dining room to prevent him from hitting anyone else. Physician and family notified and received order to transfer to another facility. R7’s closed record also contained a notice of involuntary transfer or discharge and no opportunity for a hearing. This notice was dated 10/28/2011 at 5:15PM and has a signature of E5 as the facility’s agent. This form had R7 and Z3 names regarding resident information. The proposed transfer or discharge was listed as October 28, 2011. On 3/01/2012 at 11:15AM, E1 (administrator) reported no thirty-day notice was given.”
- Failure to provide psychosocial interventions to residents who exhibit negative behaviors: “R7’s care plan with dates of 3/15, 6/06 and 8/31/2011 did not address R7’s aggressive behaviors with any type of psycho-social or activity program, prior to R7’s transfer from the facility on 10/28/2011. No social services were found addressing the aggressive behavior. On 2/29/2012 at 3:30pm, E3 (social service) was unable to identify any programs or activity R7 was
involved in to prevent any negative behaviors.”
- Failure to provide a prescribed therapeutic diet: “On 5/11/11 around 1120 AM, R12 was observed sitting in the dining room eating lunch. Suddenly she began to cough as if she was choking. Several staff present at the time ran to her aide. After R12 regained her breath it was discover that she had the wrong mechanically altered diet. R12 had been given vegetable soup as a part of her meal. The meal ticket on the tray read mechanical soft with thicken liquids and pureed fruit. The in house Speech Therapist (E4) was interviewed on 5/12/11. When asked what the result could be for a resident who is on a pureed diet that receives a mechanical soft diet, she stated that,” it could lead to aspiration.”
- Failure to provide medical reason for the use of an indwelling catheter: “On 5/10/11 between 9:35 a.m. to 10:10 a.m. E7 stated there is only one indwelling catheter on the 3rd floor and it is R1. E7 stated the reason for the indwelling catheter is because R1 has excoriation to the peril-area and it is to promote healing of the excoriation. On 5/12/11 at 10:12 a.m., E8 (nurse aide) stated she has not yet given incontinent care to R1 but is going to do it now. Surveyor asked if R1’s indwelling catheter was in place and E8 did not respond. As R1 is placed into bed by E8 and E14 (nurse aide), it is noted R1 no longer has an indwelling catheter and the back side of R1’s pants are wet. After R1’s pants are removed, E14 asked E8 if the pants were wet. E8 stated “no”. Surveyor asked if she was sure. E8 quickly looked at pants and rolled up them up and placed into plastic bag. As R1’s diaper was being removed, it was heavily saturated with urine, brown discoloration and black tarry fecal matter. Review of the bowel and bladder assessments (5/5/11, 2/15/11 and 11/15/10) document there is no medical reason for the use of an indwelling catheter. On 5/13/11 at 9:58 a.m. with the administrative staff, E3 (director of nursing) stated the order for discontinuation of the indwelling catheter was obtained after it was brought to their attention there was no order seen for discontinuation and R1 no longer had the catheter.”
