In a summary statement of deficiencies dated 11/19/2015, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "report an allegation of abuse immediately to the Administrator, delaying the initial investigation of potential abuse for three months.” The deficient practice by the nursing staff and Administrator at Integrity Healthcare Wood River affected one resident at the facility.
A part of the investigator’s findings involved a Group Interview Meeting with the resident at 9:00 AM on 11/17/2015 who stated that “I was sleeping by the piano three months ago, and [a Certified Nursing Assistant (CNA)] pinched me on the shoulder to wake me up. I told [a Licensed Practical Nurse (LPN)] that the CNA pinched me on the shoulder.” During the discussion, the LPN “checked my shoulder out, and told me [the resident] that she [the LPN] didn’t see any spots on my shoulder.” In response to the discussion with the LPN, the CNA “came back and asked me why did I scream and was I trying to get her in trouble.”
The Licensed Practical Nurse was interviewed the following day at 1:00 PM and stated that the resident “told me about three months ago that [the CNA] scared her when she woke her up.” The resident “did not tell me that [the CNA] woke her by pinching her. [The resident] told a [Registered Nurse (RN)] that the CNA pinched her [...and that the RN] reported it (the pinching allegation) to me.”
A review of the resident’s medical records “did not document any information about this allegation of abuse. The facility had no documentation that an investigation had been completed regarding this allegation.” The investigator interviewed the facility Administrator at 3:45 PM on 08/17/2015 who stated: “I am not aware of this allegation, but I will investigate it now.”
- Failure to Develop, Implement and Enforce Policies regarding Abuse and Neglect at the Facility
In a summary statement of deficiencies dated 11/19/2015, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility's failure to "operationalize its abuse policy by not reporting an allegation of abuse immediately to the Administrator, delaying the initial investigation of abuse.”
The investigator noted the failure of the nursing staff and administration to follow established procedures and protocols concerning an allegation of abuse in accordance with the law. This deficient practice failed to follow the facility’s policy titled Abuse Prevention Program Facility Procedures that reads in part:
“Employees are required to report any incident, allegation or suspicion of potential abuse, neglect, mistreatment… They observe, hear about, or suspect to the Administrator immediately. The supervisor shall immediately inform the Administrator… all reports of incidences, allegations or suspicions of potential abuse…”
“The nursing supervisor is responsible for reporting to the Administrator.”
