The deficient practice by the nursing staff at Hickory Point Christian Village involved a review of the resident’s 12/04/2014 Care Plan calling for “intervention for [the resident to have a magnet alarm on [her] chair to alert staff when she is up without assistance.” The resident’s 5:21 PM February 15,2015 Nurse’s Notes documents that the resident “had a fall today around 1600 [4:00 PM].” At that time the resident “was toileting herself and lost balance and strength in her legs.” Nurses Notes documented later that evening at 9:54 PM that “since her [the resident] fall this evening,” the resident “complains of sharp shooting pain down her forearm.” The nurse notified the resident’s physician who “wants an x-ray done tomorrow [02/06/2015].” The radiology report documented that the resident suffered a radial fracture on her forearm.
The state investigator interviewed the facility’s Administrator at 1:05 PM on 03/17/2015 who stated that the resident “attempts to transfer herself and that [the resident’s] magnet alarm is to alert staff when [the resident] is up by herself.” The Administrator also confirmed that the resident’s “magnet alarm was not on her wheelchair at the time of her fall on 02/05/2015.”
