In a summary statement of deficiencies dated 02/18/2016, a notation was made during an annual licensure and certification survey by a state investigator concerning the facility's failure to "immediately report an allegation of abuse.”
The investigation involved a review of the facility 10/20/2015 Final Abuse Investigation Report that noted that on 10/20/2015, was reported that [the resident] was speaking to her son on the phone and stated ‘They are rough with me’.” The state investigator also reviewed the investigative packet involving the abuse incident that included interviews with the resident’s son, the facility Administrator, a Licensed Practical Nurse at the facility in two certified nursing assistants. The investigative interviews were conducted the day following the alleged abuse.
The facility Administrator stated that the resident’s son “called and informed [the Administrator of the resident’s] concern being treated roughly.” The son had “received a call from [the resident] on 10/20/2015 stated that they were rough with the resident.” One of the certified nursing assistants witnessed the statement when the resident was speaking to the sun over the phone expressing that “the girls [the Certified Nursing Assistants] are rough.” When the Certified Nursing Assistant asked the resident what that meant, the resident declined to give any details.
- Failure to Develop Policies that Prevent Mistreatment, Neglect or Abuse of Residents
In a summary statement of deficiencies dated 02/18/2016, a notation was made by a state investigator during an annual licensure and certification survey concerning the facility's failure to "operationalize their abuse policy by failing to immediately report an allegation of abuse.”
The state investigator noted that the deficient practice by the nursing staff at Heritage health – Gibson City failed to follow established procedures and protocols and failed to follow the facility’s 04/23/2014 Policy and Procedure regarding Abuse and Neglect, Involuntary Seclusion, Misappropriation of Resident Property and Injuries of Unknown Origin that reads in part:
“The facility employee or agent who becomes aware of alleged abuse or neglect of a resident shall immediately report the matter to the facility Administrator.”
