The resident was later observed to have “a large greenish-purple bruise on her left cheek.” Additionally, even though the resident was injured, it was noted that “the staff did not do neuro- checks as per facility policy.”
In a second summary statement of deficiencies dated 01/15/2015, an investigation was opened against the facility’s failure “to implement fall interventions, during a transfer, for [a resident] reviewed for falls.” This is in response to a resident falling “during a transfer with one staff from the toilet to the wheelchair, hitting the right side of the four head and resulting in a laceration to the right side of the four head. The same report documents, ‘restorative nurse reassessed transfers and [the resident] changed to a two person assist’.”
