In a summary statement of deficiencies dated 01/10/2013, during an annual licensure and certification survey, a notation is made concerning the facility’s failure “to follow infection control measures regarding handwashing and wound care for [3 residents].” This is in response to repeated incidences where a certified nurses aide “did not wash his hands before applying gloves.” In one incident, the CNA failed to wash hands before applying the gloves and “then wiped [a resident’s] buttock clean after a bowel movement. [The CNA] threw away his soil gloves and did not wash his hands or apply hand sanitizer. [The CNA] then touched [the resident’s doorknob and wheelchair handles and then left [the resident’s] room and entered [another resident’s] room.”
- Failure to Provide a Residential Environment Free of Accident Hazards:
In a summary statement of deficiencies dated 09/09/2014 a complaint investigation was opened against Heddington Oaks due to its failure “to prevent access of medications for [2 residents] reviewed for cognitive impairment/wandering.” This is in response to a daughter of a resident finding “pills on the nightstand in [the resident’s] room, and [the resident] did not take or swallow the medications.” On a separate occasion, the facility’s director of nursing made a statement indicating “found all of [the resident’s] 7:00 AM pills in a medication cup in [the resident’s] room around 4:00 PM. The medications had been there since around 7:00 AM.”
