Disquieting Discoveries Regarding Patient Care at Heather Health Care Center

  • Failure to protect residents from abuse: “On 2/23/11 at 1:35 PM, E11 (CNA) was observed providing direct care for R2. R2 had wasted urine from the urinal in the bed. E11 was observed to enter R2’s room, and pushed R2 on his right side. E11 was observed failing to explain to R2 what E11 was about to do, failed to draw the privacy curtains or close the door prior to removing R2’s top bed covers and exposing R2’s buttocks. E11 was further observed to roughly roll the incontinent pad from under R2. E11 obtained a wash basin and towel from the bathroom and began to vigorously scrub R2’s buttocks area. R2 screamed out in a loud voice “Oh, that’s hurting me” during the course of the care. E11 stated “Well, I have to clean you up.” R2 began moaning and making facial grimaces. Then E11 was observed to pull the soiled linens from under R2 very roughly. R2 continued to scream out as he jerked from side to side “Have mercy on me.” E11 gave no further explanations to R2 as she exited the room with the soiled linens. R2 stated when interviewed after receiving care that “They throw me around in here. When I complaint they don’t listen. I want to get out of here, I was in a nursing home years ago but was never treated so roughly like I’m treated here.” R2 stated that he was in pain and the nurse call light was pulled. E11 stated in an interview following providing care. E11 was interviewed concerning the verbal responses and rough handling of R2. E11 responded “Oh well”.”
  • Failure to follow abuse protocol: “On 2-23-2011 at 1:05 PM, R10 was observed propelling self in a wheelchair very slowly down the hallway from the dining room very slowly. R10 stated to the surveyor that she needed some help from the
    staff to get to her room. E10 (nurse aide), when asked if someone could assist R10 to her room stated in response “I’m busy with someone else“. At 1:30 PM on 2/23/11, R10 requested to have assistance to go to the bathroom. E10 was observed to enter the room and assist R10 to the bathroom. E10 stated to the surveyor that R10 had recently returned from the hospital and was a little weak but that R10 could do things for herself. R10 dropped her head. R10 stated when interviewed at this time “They take a long time to answer the call light, and this is a long hallway to push back and forth from, I need help. They don’t listen and tell me that I can do things by myself.” R10 stated to the surveyor that what E10 stated was upsetting to R10. During an abuse interview with E10 (nurse aide), on 2/23/11 at 3:00 PM, “Verbal abuse was when you yell and scream at a resident, mental abuse is when you hurt someone mentally, like punishing them.” On 2-24-2011 at 9:40 AM, E10 was observed on the unit providing direct resident care. On 2-24-2011 at 12:20 PM E10 was observed assisting E13 (nurse aide) to provide care for R2. E1 (administrator) stated when interviewed on 2/24/11 at 10:00 AM, that it is the facility’s policy that when a staff member has an allegation of mistreatment/abuse leveled against them, the staff member is sent home until an investigation into the allegation has been completed. E1 was made aware by the survey team that E10 had been observed on the unit providing resident care.”
  • Failure to ensure proper care to residents with Gastronomy tube feedings: “On 2-24-2011 at 12:20 PM R2 stated to E7 that his bed was wet. E13 and E10 (nurse aide) entered the room and lowered the head of R2’s bed and began to roll R2 while changing the linens. Surveyor observed that R2’s head of the bed was in the flat position with [MEDICATION NAME] 1.2 tube feeding infusing at 120 cc per hour. On 2-25-2011 at 3:15 P.M. E 7 (nurse) was interviewed and stated that the tube feeding should be turned off by the nurse before the head of the bed is lowered to prevent aspiration.”
