- Failure to Follow Protocol to Prevent the Spread of Infection Facility-Wide:
In a summary statement of deficiencies dated 08/05/2014, a complaint investigation was opened against Heartland of Peoria for its failure “to prevent cross contamination during pressure sore treatment for [2 residents].” This is in response to an incident involving a registered nurse and wound care nurse who “cleansed [the resident’s] heel wound. Wearing the same contaminated gloves, [the registered nurse] tore piece off of the collagen matrix sheet and applied the torn piece to the wound bed of [the resident’s] heel.”
- Failure to Provide an Environment Free of Accidents:
In a summary statement of deficiencies dated 01/21/2014, a complaint investigation was opened against the facility concerning its failure “to follow the individualized care plan and failed to update the care plan after a fall for [a resident].” This is in response to a resident who requires assistance during transfer who was “found sitting on buttocks side wife’s bed, raised head (area) to the right side of the head noted approximately 5.5 cm – about the size of a golf ball.” The resident’s care plan was updated after the unsupervised event “with an intervention of ‘slippery socks’.”
