According to the resident’s criminal history analysis, the resident “is an Identified Offender with Mental Illness/Substance Abuse and a moderate risk, requiring closer supervision and more frequent observation than standard or routine for most residents in an open facility.” In addition, the undated care plan of the resident “does not contain focus, goals or interventions related to [the resident’s status]”.
In a summary statement of deficiencies dated 07/14/2015, a complaint investigation was opened against the facility for its failure “to implement increased security measures related to escalated exit seeking behaviors for one resident.” This is in response to the resident entering “a stairway and was found sitting at the base of the stairs with injuries (closed head injury; hematoma of frontal scalp). The facility door alarm system failed to alert staff that [the resident] had entered the stairway. This has the potential to affect at risk residents [at the facility.]. This failure resulted in An Immediate Jeopardy.”
