On 1-11-2012 at 3:51PM in the conference room, E9 (Nurse) stated, “I apologize, I made a mistake and gave [MEDICATION NAME] to R3 and I gave [MEDICATION NAME] a few hours earlier. I did not realize it until I readmitted R3 with a narcotic overdose diagnosis, I then realized that I gave the wrong medication on 11-23-2011 and R3 went to the hospital on 11-24-2011. I reported the medication error on 11-30-2011.”
  • Failure to properly prevent and treat pressure sores: “Based on interview and record review facility failed to 1) do an assessment for pressure sore 2) develop a care plan for pressure sore and 3) get physicians treatment order for pressure sore for R15. Record review of treatment record for R15 dated 9-16-10, gauze to right sacral area, [MEDICATION NAME] daily, and cover with dressing denotes that nurse signed off from 9-16-10 to 9-29-10. Record review of physician ‘ s orders in the clinical records dated 9-1-10 to 9-30-10 no order for gauze dressing to right sacral area, [MEDICATION NAME] dressing daily, cover with dressing. Record review of treatment record for R15 for 9-1-10 to 9-30-10 has no nursing written comments, description, width, length, and depth on the front or backside.” Read more about bed sores in nursing homes here.
