I reported the medication error on 11-30-2011.”
- Failure to properly prevent and treat pressure sores: “Based on interview and record review facility failed to 1) do an assessment for pressure sore 2) develop a care plan for pressure sore and 3) get physicians treatment order for pressure sore for R15. Record review of treatment record for R15 dated 9-16-10, gauze to right sacral area, [MEDICATION NAME] daily, and cover with dressing denotes that nurse signed off from 9-16-10 to 9-29-10. Record review of physician ‘ s orders in the clinical records dated 9-1-10 to 9-30-10 no order for gauze dressing to right sacral area, [MEDICATION NAME] dressing daily, cover with dressing.
