As a result, the resident “jumped out the fourth floor dining room window in a suicide attempt and was found by police on patrol at approximately 4:07 AM.” It was also noted that “the failure to monitor and supervise this resident led to an immediate jeopardy.”
- Failure to Maintain a Safe and Sanitary Residential Environment:
In a summary statement of deficiencies dated 06/25/2008, a complaint investigation was opened concerning the facility’s failure to provide residents a safe and sanitary environment. This includes a failure to “maintain the proper maintenance of equipment being used by residents of the facility.” This complaint is in response to a tour of the facility by an on-site surveyor who noticed that the washer was not working on the sixth, fifth, third and second floor and that the dryer was not working on the sixth and fifth floor.
In addition, it was also noted that the “non-operation of the washers and dryers caused the residents hardship in that they were not able to maintain grooming due to the inability to wash and dry their clothing.”
