The incident was first noted after a falling incident occurred at the facility where the resident was discharged to a local hospital. The state investigator reviewed the resident’s MDS (Minimum Data Set) that revealed the resident “was not steady, only able to stabilize with staff assistance for moving from seated to standing positions, walking, turning around and moving on and off the toilet and surface to surface transfer.” The resident’s Fall Risk Assessment showed that the resident scored an 11 indicating the resident “was a high fall risk with a score of 10 or above.”
On the date of the incident, a Certified Nursing Assistant transferred the resident “to the bathroom with a walker [… by bringing] a wheelchair in to transfer [the resident] to the bathroom but [the resident] said she could walk with a walker.” The Certified Nursing Assistant said that “she did not place a gait belt on [the resident] during the transfer [...and] said she walked with the resident to the bathroom and let go of [the resident] to adjust the toilet seat.” The CNA said that “when I let go of her, she fell… on her buttocks and then fell backward and hit her head on the wheel of the wheelchair.”
The state investigator noted that the action of the Certified Nursing Assistant failed to follow the facility’s policies and procedures titled Safe Resident Handling that reads in part: “Gait belt usage is mandatory for all resident assists with the exception of bed mobility and/or medical contraindications.”
