Troubling Findings Regarding Patient Care at Glenview Terrace Nursing Center

A survey conducted by the Illinois department of health and other private health Inspection organizations (ProPublica), unearthed numerous episodes of poor patient care, frequent falls and poor supervision that could be labeled under the term of nursing home negligence. Our nursing home attorneys were contacted to review these findings and we came up with the following facts/conditions that highlighted the possibility of negligence on the part of Glenview Terrace Nursing Center:

  • Failure to provide interventions to prevent falls: “The facility failed to implement an alternate monitoring device for R1, who made constant attempts for unassisted transfers and was known to disconnect herself from the existing mobility alarm. This failure resulted to R1’s unassisted ambulation that resulted to a falling incident on 7/5/09. R1 was sent to hospital precipitated by the fall incident and was admitted with a fracture on the left ankle.”
  • Failure to effectively monitor residents identified as wanderers: “Nursing assessment dated 7/7/09 showed that R2 was a high risk for elopement and also a wanderer. The facility failed to ensure that R2’s monitoring device was not defective. This failure resulted to R2’s elopement that occurred on 7/8/09. R2 was found by the police department along the street of a shopping mall approximately 2.21 miles away from the facility. R2 had to pass by one of the three possible road routes in which two of the three routes are major streets and has multi-lane traffic vehicles.”
  • Failure to provide accessible call lights: “January 25, 2011 at 10:20 a.m. during the initial tour (R7) was observed in his bed. The door was closed shut and the privacy curtain completely pulled around his bed. (R7)’s left (L) and right (R) arm
    and hand is severely contracted, he is able to make verbal sounds without difficulty. The call light was placed on the back of the headboard, out of reach for (R7) if he needed the assistance of staff. The call light in (R7)’s room was not appropriate for a resident who is unable to hold and press a button.R33 is alert and oriented was in bed. R33 complained of leg pain. When the surveyor asked if she notified the staff, R33 said she cannot find her call light. Surveyor observed the call light on the floor behind the head of bed.”
  • Failure to implement pressure ulcer interventions to prevent bed sores from forming : “On 1/26 /11 at 12:55 pm, R4 observed in bed with incontinent pad saturated with urine down to the draw sheet. Brown traces of urine were observed by surveyor on the draw sheets. The dressing was crumpled and wound exposed; wound appeared pale from exposure to urine for a long time. Z1 stated R4 just finished her therapy and refused to be changed. Surveyor informed Z1 that R4 was extremely soaked with urine and the brown ring shows that R4 has not been changed in a while and it’s not acceptable. Z1 stated that the urine is not that old. Z1 started to clean R4 with the help of E22 LPN. The facility failed to accurately assess the wound, failed to inform the physician of wound progress to update current treatment regimen, and failed to enhance and promote healing by ensuring R4’s wound was not exposed to urine for long periods of time.”
