Concerning Findings Regarding Patient Care at Glencrest Healthcare & Rehabilitation Centre

A survey conducted by the State Department of Health and diverse private health inspection firms un-earthed some episodes of poor patient supervision and inadequate patient transportation which led to patient falls and other avoidable health issues. The survey conducted by ProPublica—a private inspection firm—was reviewed by our nursing home attorneys in Rosenfeld Injury Lawyers and episodes of poor supervision which could be construed as nursing home negligence were found:

  • Failure to follow physician’s order and/or notify physician that a patient is refusing treatment:  “On 5/13/10 at 11:00am, R21 was observed asleep in bed with [DEVICE] infusing; the oxygen per nasal cannula was not administered.
    On 5/13/10 at 11:15am, E25 (RN) stated: “R21 always refuses the oxygen by removing the nasal cannula.” When asked by surveyor if the physician was notified of refusal and if refusal was care planned, E25 stated: “no.” At 11:20am, E25 administered the oxygen by nasal cannula to R21.”
  • Failure to take precautions to prevent the development of new pressure sores or to help existing sores heal: “On 5/13/2010 at 11:20 AM, surveyor accompanied the treatment nurses E20 and E22 to observed R7’s wound care. After R7’s dressings were changed, surveyor observed that R7’s was on an air mattress. R7 had a sheet, and a draw sheet folded over 4 times. R7 was wearing a cloth diaper and along with several folds of linen had the potential to make R7’s air mattress ineffective. Review of R7’s Pressure Sore Assessments documented that R7’s had 3 stage IV pressure sores on his sacrum, upper and lower extremities.”
  • Failure to implement interventions to prevent falls: “During initial tour on 5-11-10 between with E14 (Acting Social Service Director), the hand rails were observed loose adjacent to room [ROOM NUMBER]. During the environmental tour on 5-13-10 with E1 (Administrator), E28 (Maintenance Director) and E29 (Maintenance Personnel), the handrail was loose adjacent to room [ROOM NUMBER].”
  • Failure to develop a fall prevention plan and fall risk assessment: “A review of R41’s clinical record R41’s care plan for fall was developed on 11/5/10 after R41’s fall incident of 10/25/11. E2 (director of nursing), said that fall risk assessment are completed quarterly, and after each fall incident. On 3/25/11, and 3/30/11 during the daily status meeting E2 was unable to verbalize why there was no plan of care developed for R41 after being identified as being at risk for fall, and why the quarterly fall risk assessment was not done as indicated in the facility’s policy. According to the facility’s fall prevention policy fall risk assessments are completed at admission, quarterly and after each fall incident.”
