- Failure to follow physician’s orders: “During initial tour of 1 East floor on 8/17/10 between 10:50 AM and 12:30 PM with E4 (Registered Nurse) & E5 (Registered Nurse), R11 a bed-fast ventilator dependent resident was observed with moderately thick creamy colored urine output in the tubing. On 8/18/10 at 12:30 PM, E3 stated that E8 (Treatment Nurse) was responsible for the gastrostomy and indwelling catheter care daily and as needed. E3 stated that E8 changes the indwelling catheter monthly and as needed. E3 stated that due to R11’s condition, he has a lot of urine sediments. On 8/19/10 at 9:00 AM, R11 was observed again with very thick, beige colored urine output. Review of the July and August 2010 Treatment Records indicated that R11’s indwelling catheter and bag were changed on 7/15/10, 7/21/10 and 8/15/10. The record did not indicate that the catheter was irrigated as needed.”
- Failure to provide adequate supervision for residents: “The facility’s accident/incident report dated 6/15/12 documents that on 6/14/12, R17 went on an outing with the facility to a local outdoor recreational park and became separated from the group. This led to the local police filing a missing persons report. R17 was not located prior to the group’s return to the facility. E6 stated that on 6/14/12 she escorted residents on an outing to the recreational park. E6 stated there were 3 other staff (E5, E8-activity aids, and E9-certified nursing assistant), as well as a total of 17 residents attending the outing. E6 stated that she was in charge of monitoring 7 residents. E6 stated that when they arrived to the park, R17 had to use the rest room. E6 stated she escorted R17 to the restroom. E6 stated to the survey team she entered the restroom with R17, but soon exited because it was crowded. E6 stated that R17 did not return from the restroom after being in there for a long period of time. E6 stated that E6 did not know if there was another exit because E6 did not search for one. On 7/5/12 at 1:50pm E2 (Director of Nursing) stated that the police located R17wandering near a strip mall close to the local hospital.”
- Failure to ensure expired medications are disposed of properly: “On 8/18/10 at 11:15 AM the following were observed: Two vials Procrit-expired 11/09; 11 (22 gauge) IV cath needles with expiration date 9/2009; 21 (20 gauge) IV cath needles with expiration date 2/2010; Fleet enema-expired 2/19/08; One Acetaminophen suppository-expired 6/2010.”
